Our PCN
We are the second largest PCN in Dorset serving a population of 67,000 living in central Poole and the smaller towns of Hamworthy, Upton and extending to the rural communities around Lytchett Matravers.
Our population has a mixed demographic of urban and rural communities with varying degrees of affluence and deprivation, a frail older population living alongside younger families and those with close local communities while others are experiencing isolation and loneliness.
Our aim to is work with our partners in health and social care to develop a strong sense of neighbourhood, to listen and engage with the public voice representing our population and to work collaboratively, sensitively, and creatively to improve access to our services, health and wellbeing outcomes and, to be a focal point for our local communities.
