ENHANCED CARE TEAM HEALTHCARE ASSISTANT
Poole Central Primary Care Network is a group of 5 GP Practices in Poole, Dorset, covering a population of nearly 65,000. The practices are working collaboratively to create, joined-up, patient centred, community-based care and support for some of our most vulnerable patients and their carer’s.
Poole PCN are keen to recruit a HCA to join our established Enhanced Care Team, which includes experienced nurses, paramedics, clinical pharmacists and administrative roles working within the primary and community care setting in Poole Central Primary Care Network. This versatile team deliver proactive care for the frail patients and those with complex needs from across all 5 practices in Poole Central. They work in close collaboration with the local community trust, secondary care and Social Services teams with specific focus in the areas of frailty, and complex care.
We arelooking for someone who is an excellent organiser and communicator, who enjoys working in a busy team environment. The role will involve the co-ordination of proactive, clinical case management for frail and complex patients and those with long term conditions who are at risk of further deterioration in health, or an avoidable hospital admission or unnecessary length of hospital stay. The successful candidate would demonstrate compassion and have a passion for holistic care, empowering individuals to remain as actively well as possible. It would be advantageous to be able to demonstrate a keen interest in an area/s of chronic disease management.
Hours: Full time, 37.5 hours.
Salary equivalent to Band 2/3, dependant on experience.

CLINICAL PHARMACY TECHNICIAN
Poole Central Primary Care Network is a group of 5 GP Practices in Poole, Dorset, covering a population of 65,000. The practices have been working together to develop local services and have an established integrated Enhanced Care Team. The Team provides, holistic person-focused care and support for some of our most vulnerable patients and their carers in our local community.
We have an exciting opportunity for an enthusiastic and experienced Pharmacy Technician to provide in-surgery pharmaceutical support and advice. The day-to-day work will primarily be in specific surgeries, but you may need to travel for meetings and for collaborative working with the other Pharmacy Technicians and Pharmacists in the team. Suitable training and support will be provided according to the requirements of the successful candidate.
This is an interesting and varied role, where the Pharmacy Technician’s core responsibilities will cover clinical, technical and administrative categories, to enable greater effective and efficient use of medicine. You will be at the forefront of driving this valuable expansion and collaboration of the primary care services across the five practices in the Network.
The successful candidate would have a strong ethos for team working and experience in primary care or community pharmacy desirable. A commitment to enrol on CPPE PCPEP is essential if not previously completed and support will be given to complete the course.
This role is offered on a full-time or part-time basis.
Salary equivalent to AFC Band 4/5, depending on experience and completion of PCPEP
