Help and Care are offering a drop in wellbeing event at Poole Lighthouse on the 12th September.
As part of this event, Poole Central PCN (Primary Care Network) will be holding a blood pressure drop in facility for patients registered at the following practices –
The Adam Practice, Evergreen Oak Surgery, Poole Town Surgery, The Newman Practice and Rosemary Medical Centre.
This event will give our patients the opportunity to have an updated blood pressure reading taken by one of our Health Care Professionals. We are also here for any advice for patients with Hypertension, to offer our NHS approved app BP@home.
The app gives patients the opportunity to manage Hypertension remotely by using a BP machine in the comfort of your own home. You can then input the readings into the app, which is monitored by our Clinical Pharmacy Team. Our team will create your profile and access any needs or reviews prompted by the information provided within the app.
